If you have a query about the Northern Lincolnshire Community MSK and Pain Service, you can access our FAQs below.

If, during your care, it is agreed that a referral to Secondary Care/Hospital is the best option for you, your clinician will discuss this with you to understand where you would like to be seen. Our support staff will then refer you to your chosen provider for further care using the agreed processes.
Patients will be offered the choice of face-to-face, telephone or video appointments where clinically appropriate. We expect most appointments to be delivered face-to-face but are also aware that some patients prefer telephone or video appointments at times in their treatment.
We will be looking to add location details once these have been confirmed. Many of these will be in use by the current providers.
There will be a dedicated Telephone when the service goes live. You can also email Connect Health with general enquiries via the website: https://www.connecthealth.co.uk/contact/
Patients can self-refer or be referred by your GP or healthcare professional once the service starts.
Your current provider will contact you to explain your options and process involved.
You do not need to seek a new referral into the service by your GP/health care professional. You will be contacted by your current provider to outline the transfer process into the new service.
